When Creating a Form in the Model view we want the same option as creating a form in Table view like being able to attach a document.

When Creating a Form in the Model view we want the same option as creating a form in Table view like being able to attach a document.

Right now we have to Create the Form in the Model view then go into the table view then attach the document. This is very inefficient.
